INSTRUCTIONS:

Applications for a grant from the Basilian Human Development Fund (BHDF) must be presented on the form provided and must be complete and ready for review. It must be typed or printed in English.

1. Limit your application to the space provided on this form. Significant relevant supplementary material may be submitted which will be utilized at the discretion of the committee.


2. Submit proof of Tax-Exemption with the application or a supporting official document.


3. Six (6) copies of this application are to be received by 01 April. No proposals will be accepted after this date. Do not Fax or E-mail the application.
